CMIA PRINCIPLE TEST - HIV TEST KIT GENERASI PALING LATEST

Salam sorry mencelah kat sini, sekadar share info itu pun apa yang saya tahu sahaja. Kalau anda semua perasaan dalam setiap medical report kalau ada yang buat ujian HIV lah untuk tahun 2013, bermula 1 January 2013, setiap ujian HIV akan menggunakan kaedah cmia principle test

Cmia principle test ini adalah kit test HIV generasi ke-4 iaitu generasi yang paling latest, di mana ianya ada kemungkinan untuk detect jangkitan HIV seawal seminggu selepas infection (berjangkit). 

Sebab itu bagi sesiapa yang pernah buat ujian HIV dalam tahun 2013 ini, kalau tuan/puan ada nampak pada "report hiv test TUAN/PUAN ada tertulis HIV 1 and 2 ag/ab not detected, itu maknanya kaedah ujian itu menggunakan teknik ujian generasi ke 4, di mana ujian tersebut tidak hanya mengesan antibody(ab) malah mengesan antigen(ag) yang sudah boleh dikesan seawal 4-7 hari selepas jangkitan. 

Perlu di ingatkan, antigen ini (protein yang terdapat dalam virus) tidak akan dapat di kesan lagi setelah antibody di hasilkan oleh sistem imun badan kita (puratanya antibody dihasilkan 25 hari selepas jangkitan).

Namun pada usia jangkitan menjangkau 6-7 minggu, portion antibody(ab) yang memadai sudah boleh didapatkan untuk mengesan reaksi antibody seseorang terhadap HIV, kerana cmia principle ini sangat sensitif dan ketepatannya diperakui. 

Kebanyakkan HIV specialist berpendapat ujian HIV menggunakan cmia principle boleh dianggap "konklusif" (muktamat) pada minggu ke 6 dan seterusnya selepas tarikh kemungkinan jangkitan berlaku, walaupun CDC (United States Centers for Disease Control and Prevention) dan WHO (World Health Organization) masih menetapkan ujian antibody selepas 90 hari dari tarikh ada kemungkinan jangkitan sebagai konklusif, sekurang-kurangnya ujian antibody negatif pada minggu ke 6 boleh memberi "good indicator" sebelum kita mengambil ujian antibody kedua pada hari ke 90.

Oleh itu berkaitan kes Puan Lisa ini, sekiranya dia mengambil combo test (ab/ag) pada minggu ke-6 dan di dapati negatif, itu dikira satu indicator bagus untuk beliau dan kecenderungan untuk di sah kan positif adalah lebih kurang 5% sahaja

Dan katakan Puan Lisa mengambil ujian antibody sahaja pada hari ke 90, itu di kira sudah konklusif, untuk tempoh menunggu sehingga 6 bulan, itu adalah amat jarang, kebiasaan window period untuk tempoh 6 bulan berlaku pada pesakit kanser, hepatitis C. 

Ujian antibody selepas 90 hari sudah boleh di anggap konklusif (muktamat) asalkan tiada sebarang akitiviti berisiko dilakukan sehingga ke tarikh tersebut. 

Dunia semakin maju, HIV test kit pun semakin sensitif dan banyak test kit yang mampu mengesan hiv lebih awal berbanding beberapa generasi HIV test kit terdahulu. 

Tentang vaksin PEP (Post–exposure prophylaxis), itu hanya berkesan dalam 72 jam selepas waktu jangkitan, itu pun belum 100% boleh berjaya, selepas jangka waktu tersebut hanya boleh lakukan ujian DNA viral load, combo test ab/ag atau antibody.

Apapun saya doakan semua PLHIV sentiasa sihat-sihat, jangan lupa makan ubat, kami terima anda seadanya. Dengan ilmu pengetahuan, hidup mereka boleh sama dengan orang normal. No such big deal, yang penting ilmu dan peka tentang kesihatan kita.

Hello , me again , Hi, im from malaysia , age 28 male , my english is very poor hope you understand. I had a high risk unprotected with a girl work at KTV 81 days ago , i got tested for HIV at 57 days at a LAB , my result came back ( HIV 1&2 ab/ag not dedected based on CMIA principle) . And I tested by ez trust rapid test at 72 days and 80 days all negative . I have no any HIV symptoms , may i know what is CMIA principle testing metho ? Can consider conclusive ? Should i go test again at 84 days or 90 days again .
i just get my 84 days tested result came back (HIV I & II (AIDS) . Not Detected . Screening Method By ELISA) .

I'm going to do my best to answer your questions in the order you asked them above. Firstly, The Chemiluminscence (CMIA) HIV test is extremely accurate and reliable, and most CMIA tests include detection of the p24 antigen, which allows for earlier HIV infection detection. This is because an exposure which leads to an infection can take up to 3 days & P24 antigens are detectable immediately after infection, until your body's antibody response takes over (at which time the antibodies bind to the antigens, making them no longer detectable). However, not all CMIA tests may include p24 antigen detection, so retesting at 12 weeks is often suggested. You went on to receive two more HIV tests, all of which were negative. Your results are indeed conclusive and I wouldn’t suggest further testing.
So to clarify, the first test that you have with the results: ”HIV 1&2 ab/ag not detected” was looking for both the antibody (ab) and the antigen (ag). This is only available during the first couple weeks, as once your body starts producing the antibodies the antigen is no longer detectable. This is why the tests you had to follow did not say (ab)/(ag), as it was only looking for the antibodies.
That being said, all the testing methods provided are considered completely conclusive at the 3 month mark following an exposure. Your 84 day test results are 100% conclusive and you are indeed HIV negative. I encourage you to try not to focus so much on the science behind the testing and focus on the negative test results.
